Nurse Practitioner

Blize Healthcare is looking for an experienced and hard-working Nurse Practitioner to join their growing team.

Job Description:
Home health nurse practitioners (NPs) work with patients and their families at their homes to provide health care and advice on how to maintain healthy lifestyles. They travel to patients’ homes when they are ill or unable to travel, often serving the elderly and disabled. In addition to providing primary care, these NPs may also supervise and train home health aides, provide preventative care, and help families create plans for diet, exercise, and medication.

Minimum Requirements include:

• Must have graduated from an accredited school.
• Must have minimum 1 year of experience as NP
• Current state license as a Nurse Practitioner
• Current CPR certification.
• Provide skilled nursing services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of skilled nursing interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
